Most physicians and nurse practitioners who have never done BoNT treatments for their patients worry that if they start providing BoNT in their offices, they may make someone look weird. Or, not being plastic surgeons, they may worry that their patients will not trust them to do Botox.

Yet, many people want BoNT treatments, and doing BoNT (or other neuro-modulators) can both make people happy and help supplement a physician's income, making it easier for the physician to spend time with complicated medical patients without going broke.

Also, BoNT can be used to treat some very difficult-to-treat MEDICAL problems--but we will get to that later in this memo.

How do I know?

I trained in internal medicine and then (after 12 years in the ER) opened up a private internal medicine practice in February 2000. So, I know the following to be true: If you really spend enough time to take excellent care of an 82-year-old man taking 12 medications and suffering from chronic heart failure, renal failure, and an acute respiratory infection, if you really spend enough time with that man to take excellent care of him, then the cost of your overhead for the hour you spend with him will be more than what you will be paid by insurance for making life-and-death decisions.

But, if you then, on the next patient, do a BoNT treatment, you will make sufficient profit with the BoNT that the two visits combined will make enough money to easily pay your staff, your malpractice insurance, your light bill, and still have some leftover for gas for your car and groceries for your family.

In other words, doing BoNT (and other cosmetic procedures) can help finance the time required to care for the seriously ill.

Put more plainly, your 35-year-old healthy woman will finance your care of the 75-year-old chronically ill woman.

Also, unlike many branches of medicine, assuming you have reasonable dexterity, you can learn to provide great BoNT treatments with very little risk in a very short time.

How to know if you have sufficient eye-hand coordination to do great BoNT...

Do you have a reasonable steadiness of hand (i.e. no severe intention tremor)? Can you accurately put a needle where it needs to go (i.e. if you see a large vein, can you start an iv or draw blood on most attempts)? Do you have enough vision (even using readers or loops) to read the print in a newspaper? If so, then you have the physical requirements to do BoNT.

And yet, even knowing the possibilities, most physicians repeatedly go to BoNT course after course; then, come home and still do not feel confident that they can treat their patients with Botox without making them look strange.

Or, the physician does the course (or reads the book) but has no idea how to let their patients or (people outside their practice) know how to schedule an appointment for Botox. Or, just as commonly, the physician may be so accustomed to insurance that she has no knowledge of how to ask for the cash payment for the Botox without feeling uncomfortable and embarrassed.

I went through this same struggle over a decade ago.

My Embarrassing Journey

Both inner-city and rural emergency rooms were my home for 12 years; and (like many critical-care physicians) I found it to be relaxing because I knew my business and the intensity of the arena demanded a focus that approached meditation.

If you have done critical care medicine, then you know what I speak.

Then, I picked up a BoNT syringe, and I was absolutely terrified! I was terrified that I might make someone look stupid. There was nothing relaxing to me about doing Botox; only uncertainty and fear.

Of course, I would laugh at myself about how nervous I was. I felt ridiculous about being uncomfortable because I knew that (unlike doing CPR in the ER with 32 other sick people simultaneously under my care) when one does a  BoNT treatment, there is no chance anyone will die from a mistake.

Millions of cosmetic BoNT treatments are done every year, and no one dies from them.

Even knowing this, I did not feel confident, and that made me resistant.

And Then It Hit Me!

BoNT wears off in 2-3 months, no one dies from it, and so I really just needed to simplify the multiple methods I was learning from textbooks and courses and design a way that came a close as humanly possible to guranteed beautiful results with zero risks of serious sequelae or of making someone look strange.
